Gardening for Wildlife Entry Form

You can enter by completing the form below. All entries must be received by 31st May 2008.

All entrants must send photographic evidence with their completed entry form (use mail). An assessor may contact you to arrange a visit during June or July. Award ceremonies will be organised by local authorities and take place in the Autumn.

Please tick to show that you have at least three of the following to include at least one from each column A, B and C to ensure a Bronze Award. At least two ticks from each column ensures a Silver and three ticks from each column will ensure a Gold Award.
