What's your personal experience been? Are you satisfied? Have you felt in control of what's been decided about your care?

NHS Dartford, Gravesham and Swanley Clinical Commissioning Group, which plans and pays for most of your local healthcare services, wants to know whether community-based health services for adults can work better for you.

We need to do this because more people are using the NHS than ever before – we're living longer and so have more long-term, often complex health problems.

We need to see how we can support people better so we all receive the right care, from the right person, at the right time, in the right place.

We really need to hear about your experiences so we can improve your local services.
